Social Services
Health is about more than medical care. At BMS, our Social Services team helps patients and families navigate the challenges that impact health—connecting you to benefits, resources, and support so you can focus on what matters most.
How We Help
- Benefits enrollment assistance (Medicaid, SNAP, housing support, and more)
- Referrals to community resources
- Care coordination and case management
- Help with transportation, translation, and appointment scheduling
- Connection to mental health and crisis support
- Support for new parents and families through WIC
No matter what you’re facing—food insecurity, housing instability, insurance questions, or just not knowing where to start—we’re here to help you find a path forward.
