BMS FAMILY HEALTH AND WELLNESS CENTERS WIC PROGRAM RECEIVES 

USDA GOLD BREASTFEEDING AWARD OF EXCELLENCE 

BMS Family Health and Wellness Centers Women, Infants and Children (BMS WIC) program received the prestigious USDA Gold Breastfeeding Award of Excellence 2024 for providing exemplary breastfeeding promotion and support to WIC families. The BMS WIC team previously received the award in 2015. BMS WIC is a federally funded, state-run program focused on the nutritional wellbeing of low-income pregnant, breastfeeding and postpartum individuals, infants, and children up to the age of five. Our WIC program provides participant-centered tailored food packages, nutrition counseling, breastfeeding/chestfeeding promotion and support, and social service referrals to eligible families in communities we serve. 

 

 

NCQA Awarded BMS with 2023-2024 NYS Patient-Centered Medical Home 

BMS received our 2023-2024 NYS Patient-Centered Medical Home (PCMH) recognition from National Committee for Quality Assurance (NCQA) for our 3 Primary Care sites: Main, Ashford, and Bristol. 

  

PCMH is a national recognition for Healthcare Facilities (hospitals, outpatient facilities, etc.) to provide high-quality, cost-effective healthcare in a manner that is comprehensive, coordinated, and patient-centered.  This process requires an annual renewal of standards which are updated by NCQA and NYS every year starting January 1st of each year, and BMS is among the first group of healthcare facilities in New York State to receive our annual recognition. 

 

We thank our amazing BMS staff for their ongoing effort and work ethic to deliver services that empower our community to take charge of their own health.  

BMS Wins HRSA Quality Awards and NCQA Recognition

 

The Health Resources and Services Administration (HRSA) awarded BMS for its achievements in delivering high-quality services and contributions to the local community’s wellbeing. BMS was acknowledged for the quality of its clinical services, approach to patient care and experience, and the use of technology for information management.  

 

We are pleased to announce that in 2024 BMS Received the following Quality Awards: 

 

Silver Medal Award 2024: Health Center Quality Leader  

BMS was among the 12% of NYS Health Centers Awarded the Health Center Quality Leader (HCQL). This badge recognized BMS’ notable quality improvement in achieving the best overall clinical quality measure (CQM) performance among all health centers 

Advancing Health Information Technology (HIT) for Quality 2024

This award recognizes BMS optimization of HIT services that facilitate communication between health care providers; improve medication safety, tracking, and reporting; and promote quality of care through optimized access to and adherence to guidelines. To be considered for the award, BMS had to meet the following criteria: 

  1. Adopted an electronic health record (EHR) system.

  1. Offers telehealth services.

  1. Electronically exchanges clinical information with key providers and healthcare settings.

  1. Engages patients through health IT.

  1. Collects data on patient social risk factors.

 

Patient-Centered Medical Home Recognition (PCMH) 

The PCMH is a model of care that puts patients at the forefront of care, building better relationships between patients and their clinical care teams. PCMH recognition acknowledged BMS’ efforts and results in improving patient experience, and staff satisfaction—while reducing health care costs.
