What to Expect as a New Patient
1. Your First Visit
During your first appointment, our staff will:
- Review your medical history and current health needs
- Confirm your insurance information
- Discuss your health concerns and goals
- Create or update your personalized care plan
What to Bring
- A valid photo ID
- Your insurance card
- A list of current medications
- Any recent medical records, test results, or discharge papers
- Contact information for your previous healthcare provider(s)

- Ask questions and be fully informed about your treatment
- Participate in decisions about your care
- Request a second opinion
- Access your medical records
Your Responsibilities
- Provide accurate and complete health information
- Attend scheduled appointments or call ahead if you need to reschedule
- Follow the agreed-upon treatment plan
- Treat staff and other patients with courtesy and respect
Appointments & Scheduling
- Routine visits should be scheduled in advance.
- Same-day appointments may be available for urgent needs.
- If you cannot keep an appointment, please notify us at least 24 hours in advance.
Prescriptions & Refills
- Prescription refills may take 48–72 hours to process.
- Please request refills before you run out of medication.
After-Hours Care
If you experience a medical emergency, call 911 immediately.
For non-emergency questions after business hours, you may contact our on-call provider
through our main phone line.
